Submission of this form serves as your 15‑day written cancellation notice, as required by your membership agreement. By submitting the request, you confirm that you have read, understand, and agree to the terms outlined below.

Important Terms & Conditions

• 15‑Day Notice Requirement: All membership cancellations require fifteen (15) days’ written notice prior to the billing date.

• Billing During Notice Period: If a scheduled renewal payment falls within the 15‑day notice period, that payment will be processed as scheduled.

• Non‑Refundable Payments: All membership payments are non‑refundable.

• Commitment Period Cancellations: Cancellation of a membership prior to the expiration of any specified commitment period (e.g. 26 week contract, 52 week contract) requires completion of the 15‑day notice period and payment of a termination fee.

• Termination Fee: The termination fee is equivalent to the balance owed for the active membership period, calculated as the difference between the commitment membership rate and the month‑to‑month rate for all months the membership was active.

  Example: If the difference between a 26‑week agreement and a month‑to‑month membership is $15 per month, and the 26‑week agreement is terminated early, the balance owed ($15 x the number of active months) will be due upon cancellation.

• Reactivation After Cancellation: If you choose to reactivate your membership after cancellation, the membership rates in effect at the time of reactivation will apply. Membership rates are subject to change.
